Job summary

Enhabit Home Health & Hospice is seeking professionals to coordinate care for patients at home, assess needs, and implement care plans in coordination with physicians.

Must be licensed, have CPR, and drive; strong tech skills with mobile devices; Medicare/home health experience preferred.

Qualifications

• Graduate of an approved school of clinical education.
• Licensed in the state where they currently practice.
• At least one year of experience as a licensed professional.
• Basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Provide coordination of care, ensure continuous assessment of each patient’s and family’s needs, and implement the interdisciplinary care plan.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
• Ensure delivery of care is compliant with the federal and state regulations.

Responsibilities

Education And Experience, Preferred

• Experience as a licensed professional may be deferred with a review and approval from the regional and executive service line clinical leadership.
• Medicare home health or hospice experience is preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification for the healthcare provider
