• Be the Expert: Assess patients with complex health needs, develop personalized care plans, and ensure the best outcomes.
• Make a Connection: Educate and guide patients and families through their healthcare journey – helping them understand their conditions and the path to better health.
• Collaborate for Success: Work closely with a team of VNA clinicians and external providers to ensure seamless care and optimal results for every patient.
• Document with Purpose: Record and track changes in patient conditions, updating care plans to reflect their progress and needs.

Qualifications

• Registered Nurse (RN) with a bachelor’s degree in nursing (preferred).
• 1+ year of clinical experience as a registered nurse (bonus points if you have experience in homecare).
• Strong verbal and written communication skills – guiding and educating patients and families.
• Proficient with technology – navigating EMR systems and using digital tools to enhance patient care.
• A reliable car and valid driver’s license – traveling to patient homes is required.

Physical Demands / Working Conditions

• Flexibility to work both indoors and outdoors while using clinical equipment.
• Comfortable using a phone, computer, and other digital tools to stay connected with your team and patients.
• Direct client and customer contact with an impactful presence.

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $60,000 to USD $110,000. The total compensation includes base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
