Job summary
Phoenix Home Care & Hospice is seeking a skilled Home Health RN to join our compassionate team in Moberly, MO and surrounding areas. You will assess patients, interpret diagnostics, and implement individualized plans of care in collaboration with physicians and therapists.

Qualifications
- Current Registered Nurse or Licensed Practical Nurse license in Missouri.
- Home Health or Critical Care preferred.
- Driver's license and vehicle with current auto insurance.
- Ability to pass drug test, background check and physical.
Responsibilities
- Assists in development, review and revision of the plan of care.
- Provides nursing care per physician's plan of treatment and client plan of care.
- Observes, records and reports reactions to treatment and changes in condition.
- Provides ongoing instruction and supervision of client, family or health team.
- Participates in education programs, quality assessment activities and nurse meetings.
- Monitor and record patient's condition and document care via electronic charting.
