Job summary

Enhabit Home Health & Hospice, a national home-based care provider, is seeking a Full-time Weekend Registered Nurse in Colorado Springs. The role requires weekend shifts with Saturday and Sunday coverage to support patient care in the home health setting.

You will perform skilled nursing visits, coordinate care with multidisciplinary teams, and ensure proper documentation including OASIS. Patients rely on your expertise to maintain quality outcomes in a home care environment.

Qualifications

• Must be licensed RN in the state of employment.
• Minimum of one year of clinical experience preferred.
• Valid driver’s license and auto insurance required.

Responsibilities

• Perform skilled nursing visits and coordinate client care.
• Act as point of contact for all disciplines involved with care.
• Oversee the frequency of visits for the episode.
• Consult with physician and office regarding patient care.
• Perform accurate OASIS collection and maintain medication profile.
• Ensure lab values are reported to physician timely.
• Attend weekly case conference and monthly case manager meetings.

Responsibilities

Enhabit Home Health & Hospice is looking for a Full-time weekend ]RN position.

Flexible scheduling, but Saturday and Sunday are required.

The Home Health Weekend Registered Nurse is a field employee who:

• Performs skilled nursing visits and completes coordination of client care,
• Point of contact for all disciplines involved with providing care to patients
• Oversees the frequency of visits for the episode.
• Consults as needed with the physician and the office giving details about patient care.
• Performs accurate OASIS collection, ensuring the medication profile remains current.
• Ensuring lab values have been reported to the physician timely,
• Attends weekly case conference and monthly case manager meetings

Qualifications

Registered Nurses (RNs) must meet the following requirements:

1. Be currently licensed as a Registered Nurse (RN) in the state of employment
2. A minimum of one year of clinical experience is preferred
3. Demonstrate knowledge and skill in current nursing practice
4. Possess a valid state driver’s license and automobile liability insurance
