Job summary

Enhabit Home Health & Hospice is seeking a Full-time Weekend Registered Nurse to provide skilled nursing visits in the home and coordinate patient care. The role requires scheduling on Saturdays and Sundays, with responsibility for care coordination across disciplines.

The RN will perform OASIS data collection, maintain current medication profiles, and consult with physicians to ensure high-quality care. A valid RN license in the state of employment and a driving license are required.

Qualifications

• Active RN license in the state of employment.
• Minimum of 1 year of clinical nursing experience preferred.
• Valid driver’s license and automobile liability insurance.

Responsibilities

• Performs skilled nursing visits and coordinates client care.
• Point of contact for all disciplines involved with patient care.
• Oversees the frequency of visits for the episode.
• Consults with physician and office about patient care details.
• Performs accurate OASIS collection and keeps the medication profile current.
• Ensures lab values are reported to the physician timely.
• Attends weekly case conference and monthly case manager meetings.

The Home Health Weekend Registered Nurse is a field employee who:

• Performs skilled nursing visits and completes coordination of client care,

Qualifications

Registered Nurses (RNs) must meet the following requirements:

• Be currently licensed as a Registered Nurse (RN) in the state of employment
• A minimum of one year of clinical experience is preferred
• Demonstrate knowledge and skill in current nursing practice
• Possess a valid state driver’s license and automobile liability insurance
