Job summary

Mercy Home Health is seeking a skilled RN to provide in-home patient care in the community across diverse cases. You will handle admissions, assessments, teaching, and documentation, delivering high-quality care in patients’ homes.

Qualifications

• One year of experience in home health or acute care preferred.
• Graduation from an accredited nursing program is required.
• Current RN license in Missouri is required; valid driver’s license and auto insurance proof.
• BLS certification via the American Heart Association is required.

Responsibilities

• Provide day-to-day patient care in the home setting according to Mercy policies.
• Perform admissions, post-hospital assessments, recertifications, discharges and patient teaching.
• Administer IV therapy, dressing changes and site care, ensuring safety and quality care.
• Document patient care in the Epic system with timely, accurate entries.

Job description

Provides day- to-day patient care in the home setting which includes, but is not limited to: case management and total care of multiple patients with different levels of acuity, admissions to home care services, post-hospitalization assessments, recertifications, discharges, patient teaching, dressing changes, IV therapy administration, access site care and general skilled nursing according to policy and procedure, assists with other needs that may be apparent in the home to assure safety and appropriate care i.e. lifeline, meals on wheels, and instruction on public programs availability. Provides appropriate and timely documentation by utilization of the Epic computer system.

Per Diem position provides visits on a as needed basis following all department policies. Performs duties and responsibilities in a manner consistent with our mission, values and Mercy Service Standards.

  • $0.70 per mile (subject to change per Federal Reimbursement Rate)

Qualifications:

• Experience: Prefer one year of experience in a related field (home health, acute care, etc.).
• Education: Graduation from an accredited nursing program required.
• Licensure: Current RN license in the state of Missouri required. Must also have a valid driver’s license and proof of auto insurance.
• Certifications: BLS Certification required through the American Heart Association.
