Job summary

Mercy Home Health seeks an experienced RN to deliver patient-centered care in home settings, managing cases and ensuring safe, effective outcomes. You’ll perform assessments, IV therapy, and education, documenting in Epic while enjoying a flexible schedule and a supportive team.

This per diem role offers independent practice, diverse patient needs, and mileage reimbursement, with a focus on high-quality community-based care at a 5-star agency.

Qualifications

• Experience: Prefer one year in home health or related field.
• Education: Graduation from an accredited nursing program required.
• Licensure: Current RN license in Missouri; valid driver’s license and auto insurance.
• Certifications: BLS Certification through the American Heart Association.

Responsibilities

• Provide day-to-day patient care in the home setting with total care for multiple patients.
• Document care accurately in the Epic system and ensure policy adherence.

Job description

Provides day- to-day patient care in the home setting which includes, but is not limited to: case management and total care of multiple patients with different levels of acuity, admissions to home care services, post-hospitalization assessments, recertifications, discharges, patient teaching, dressing changes, IV therapy administration, access site care and general skilled nursing according to policy and procedure, assists with other needs that may be apparent in the home to assure safety and appropriate care i.e. lifeline, meals on wheels, and instruction on public programs availability. Provides appropriate and timely documentation by utilization of the Epic computer system.

Per Diem position provides visits on a as needed basis following all department policies. Performs duties and responsibilities in a manner consistent with our mission, values and Mercy Service Standards.
