Mercy Home Health is seeking a skilled RN to provide comprehensive in-home patient care across the Mercy South area. You will manage diverse patient needs, perform admissions, assessments, recertifications, and discharges, and document in Epic while following Mercy standards.

The role offers a per diem/full-time schedule with 8:00 AM-4:30 PM hours, on-call weekends, and mileage reimbursement. Missouri RN license and BLS required, with a driver’s license and insurance on file.

Qualifikationen

• Graduation from an accredited nursing program required.
• Current RN license in Missouri required; valid driver license and proof of auto insurance.
• BLS Certification through American Heart Association.
• Prefer one year of experience in home health or related field.

Find your calling at Mercy! Provides day- to-day patient care in the home setting which includes, but is not limited to: case management and total care of multiple patients with different levels of acuity, admissions to home care services, post-hospitalization assessments, recertifications, discharges, patient teaching, dressing changes, IV therapy administration, access site care and general skilled nursing according to policy and procedure, assists with other needs that may be apparent in the home to assure safety and appropriate care i.e. lifeline, meals on wheels, and instruction on public programs availability. Provides appropriate and timely documentation by utilization of the Epic computer system.

Per Diem position provides visits on a as needed basis following all department policies. Performs duties and responsibilities in a manner consistent with our mission, values and Mercy Service Standards.

Qualifications:

• Experience: Prefer one year of experience in a related field (home health, acute care, etc.).

• Education: Graduation from an accredited nursing program required.

• Licensure: Current RN license in the state of Missouri required. Must also have a valid driver's license and proof of auto insurance.

• Certifications: BLS Certification required through the American Heart Association.
