Job summary

Enhabit Home Health & Hospice is seeking a part-time Registered Nurse to provide in-home patient care in the Nashville area. The RN will coordinate care, collect OASIS data, and ensure the medication profile and lab values are current.

You will serve as a liaison among disciplines and consult with physicians and the office. The role offers 24 hours per week across four days with eligibility for our 60% paid days off program.

Qualifications

• RN licensed in the state of employment.
• Minimum of 1 year of clinical experience preferred.
• Demonstrate knowledge and skill in current nursing practice.

Responsibilities

• Perform visits and coordinate client care.
• Act as point of contact for all disciplines involved with patient care; consult with physician and office.
• Perform accurate OASIS data collection and ensure medication profile is current.
• Ensure lab values are reported timely to the physician.
• Attend weekly case conferences and monthly case manager meetings.
• Part-time salaried role: 24 hours/week over 4 days; eligible for 60% paid days off.

Responsibilities

In addition to performing visits and completing coordination of client care, the Registered Nurse RN Home Health is the point of contact for all disciplines involved with providing care to patients and oversees the frequency of visits for the episode. The Registered Nurse RN consults as needed with the physician and the office giving details about patient care. Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role. This part-time salaried position works 24 hours/week, over 4 days in the week, and is eligible for our 60% paid days off program.

Qualifications

Registered Nurses (RNs) must meet the following requirements:

• Be currently licensed as a Registered Nurse (RN) in the state of employment
• A minimum of one year of clinical experience is preferred
• Demonstrate knowledge and skill in current nursing practice
• Possess a valid state driver’s license and automobile liability insurance
