Job summary

Enhabit Home Health & Hospice, a leader in home-based care, seeks a part-time Registered Nurse to coordinate and deliver patient-centered home health services in the Nashville area. You will perform visits, collaborate with physicians and care teams, and oversee visit frequency to ensure quality care.

The role emphasizes accurate data collection (OASIS), medication review, and timely lab reporting, with weekly and monthly governance meetings to support patient outcomes.

Qualifications

• Be currently licensed as a Registered Nurse (RN) in the state of employment
• A minimum of one year of clinical experience is preferred
• Demonstrate knowledge and skill in current nursing practice
• Possess a valid state driver's license and automobile liability insurance

Responsibilities

• Perform visits and coordinate client care.
• Serve as the point of contact for all disciplines involved in patient care.
• Oversee the frequency of visits for the episode and consult with physician and office as needed.
• Perform accurate OASIS data collection and ensure medication profiles are current.
• Ensure lab values are reported to the physician timely.
• Attend weekly case conferences and monthly case manager meetings.

Responsibilities

In addition to performing visits and completing coordination of client care, the Registered Nurse RN Home Health is the point of contact for all disciplines involved with providing care to patients and oversees the frequency of visits for the episode. The Registered Nurse RN consults as needed with the physician and the office giving details about patient care. Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role.

This part-time salaried position works 24 hours/week, over 4 days in the week, and is eligible for our 60% paid days off program.
