Job summary

Enhabit Home Health & Hospice is seeking a dedicated Registered Nurse (RN) to provide in-home care in Bluffton, SC. You will assess patients, develop care plans, and coordinate with physicians and other disciplines. Key duties include accurate OASIS collection, medication review, and timely reporting. Active RN license in SC and a valid driver’s license are required.

Qualifications

• Registered Nurses (RNs) must be licensed to practice in the state of employment.
• A minimum of one year of clinical nursing experience is preferred.
• Demonstrate knowledge and skill in current nursing practice.
• Possess a valid state driver’s license and automobile liability insurance.

Responsibilities

• Perform in-home visits and coordinate client care across disciplines.
• Act as the point of contact for care details and oversee visit frequency.
• Consult with physicians and the office to relay patient care information.
• Conduct accurate OASIS data collection and ensure medication profiles are current.
• Report lab values timely to the physician and participate in care conferences.
