Job summary

Nightingale Home Healthcare seeks a dedicated Registered Nurse to deliver high-quality, one-on-one home care with autonomy and strong clinical support. Flexible schedule, competitive per-visit pay, mileage reimbursement, and comprehensive benefits are offered.

You will conduct admissions, assess patients, monitor status, and collaborate with the care team to ensure safe, effective care in patients' homes. Requires CT RN license, OASIS/WellSky experience, and valid driver's license.

Qualifications

• Current CT RN license.
• OASIS and WellSky (Kinnser) experience preferred.
• Medical-surgical and wound care experience preferred.
• TB test within 12 months and CPR certification.
• Valid driver’s license and reliable transportation.
• Willing to attend 1–3 day orientation in Stamford.

Responsibilities

• Conduct patient admissions and develop individualized Plans of Care.
• Obtain medical histories and perform physical assessments.
• Monitor patient status, vital signs, and clinical progress in the home.
• Deliver skilled nursing care in compliance with HIPAA and agency standards.
• Collaborate with the interdisciplinary team to ensure comprehensive care.
• Coordinate services with LPNs, HHAs, and therapy providers.
• Complete accurate documentation and submit clinical notes in a timely manner.
