Job summary

Piedmont Augusta Hospital is seeking a Registered Nurse for Home Health to deliver and direct nursing care in patients' homes per physician orders and agency policies. You will coordinate with physicians, staff, patients and families to ensure safe, continuous care.

The role emphasizes evidence-based nursing, compassionate patient support and collaboration within a multi-disciplinary team. Graduation from an accredited nursing program is required; no prior home health experience is necessary.

Qualifications

• Graduate from a recognized, accredited school of nursing.
• No experience required.

Responsibilities

• Provide and direct provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.
• Coordinate total plan of care with other health care professionals involved in care and help to achieve continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources.
• All done within a Point of Care setting.
