EvergreenHealth in Washington seeks a Registered Nurse to provide the total direct nursing care for home health patients, following established standards and policies. You will assess patient needs, start care, plan and evaluate care, and coordinate with physicians and families to ensure safe, high-quality outcomes.

The role emphasizes advocacy and collaboration in a dynamic home health setting. Required include WA RN license, Diploma/ADN, and current BLS; Master’s in Nursing is desirable.

Formación

• Graduate of a nursing program (Diploma or ADN)
• Licensed as a Registered Nurse in the State of Washington
• Current Healthcare Provider BLS certification by date of hire
• Proof of WA state drivers license and car insurance
• Master’s degree in Nursing (desired)

Responsabilidades

• Provides direct patient care.
• Assesses the nursing needs of home health patients.
• Coordinates patient and family education.
• Initiates start of care through admission processes.
• Plans, implements and evaluates patient care.
• Collaborates with physician and inter-disciplinary team to plan care.
• Communicates patient information and safety issues to supervisor and team.
