Job Title: Home Health Registered Nurse (RN)

Job Summary:

We are seeking a compassionate and autonomous Home Health Registered Nurse (RN) to manage and deliver comprehensive home-based nursing care. In this role, you will perform initial patient assessments, develop individualized plans of care, complete OASIS documentation, and coordinate with interdisciplinary care teams and Certified Nursing Assistants (CNAs) to ensure optimal health outcomes for patients in their homes.

Key Responsibilities:

• Comprehensive Patient Assessment: Conduct initial and ongoing physical, psychosocial, and environmental assessments for home care clients across various life stages.
• Care Planning & Discharge: Initiate, evaluate, and update patient plans of care in collaboration with physicians and interdisciplinary teams; establish comprehensive discharge strategies.
• OASIS & Regulatory Documentation: Complete accurate OASIS data collection and maintain thorough, timely electronic clinical documentation adhering to state and federal home health guidelines.
• Patient & Caregiver Education: Assess educational needs of patients and families, implementing personalized teaching strategies for disease management, medication administration, and home safety.
• Clinical Supervision: Supervise Certified Nursing Assistants (CNAs) and coordinate care delivery with therapists, social workers, and other healthcare providers.

Qualifications & Requirements:

• Clinical Experience:

  • Prior Home Health nursing experience [Required] (Strong Medical-Surgical nursing background with home health exposure will be considered).
  • Direct experience with OASIS assessments and Start of Care (SOC) documentation [Strongly Preferred].

• Licensure & Logistics:

  • Active Registered Nurse (RN) License [Required].
  • Valid Driver’s License, reliable personal vehicle, and active auto insurance [Required].
• Certifications: Active Basic Life Support (BLS) Certification [Required].
• Core Competencies: Independent clinical decision-making, time management, strong OASIS charting skills, patient advocacy, and effective interdisciplinary communication.
