Job summary

NEW SOLUTIONS is seeking an RN Home Health Clinical Manager in San Diego, CA. The role oversees patient care services, manages clinical staff, and ensures care is delivered safely and in compliance with state and federal regulations.

Responsibilities include coordinating patient care, leading quality improvement initiatives, managing OASIS/discharge data, and maintaining survey readiness. A valid RN license and CPR certification are required, with travel to multiple locations as needed.

Qualifications

• Graduate from an approved school of professional nursing.
• 5 yrs RN experience with 2 yrs or more supervisor experience in Home Health.
• Licensed to practice as a registered nurse in the state.
• Must possess and maintain valid CPR certification.
• Must be a licensed driver with an automobile and be able to travel to all business locations.

Responsibilities

• Oversee day-to-day clinical and administrative office operations.
• Coordinate patient care and supervise clinical staff.
• Direct care delivery to align with standards and regulations.
• Lead QAPI initiatives and track quality indicators.
• Maintain survey readiness and manage HHCAHPS survey distribution.
