CenterWell Home Health in Austin, TX, seeks a Clinical Manager to lead a team of clinicians delivering compassionate home-based care. You will guide clinical practice, coordinate patient services, and ensure excellence across the care journey to empower patients toward independence.

Requirements include an active RN license, home health experience, OASIS exposure, CPR proof, and a valid driver’s license with reliable transportation. The role includes a $10K sign-on bonus and on-site operations.

Qualifications

• Current state RN license is required.
• Home health care experience.
• OASIS experience preferred.
• Proof of CPR.
• Valid driver's license and reliable transportation.
• Minimum 2 years as RN with at least 1 year in management in home care or similar setting.

Responsabilités

• Oversee clinical operations for the location and in-office staff.
• Review referrals and ensure admission appropriateness and Plans of Care.
• Guide and educate clinicians; support care planning and clinical decisions.
• Ensure documentation, audits and billing meet Medicare and payer standards.
• Participate in hiring, training, coaching, and performance evaluations.
• Coordinate care communication among physicians, team members and caregivers.
• Contribute to quality improvement, budgeting, marketing and community relations.
• Provide direct patient care in exceptional circumstances and act as Branch Director in absence.
