Job summary

ProHealth Home Health and Hospice is seeking a Home Health Case Manager (RN) to provide skilled nursing care to patients in their homes. As a part of the team, you will make evaluations, prepare care notes, and coordinate services to ensure optimal patient outcomes.

This role requires a strong nursing background, excellent communication skills, and the ability to take initiative. Interested candidates must be licensed in Alabama and ideally have prior nursing experience.

Qualifications

• Two years of nursing experience preferred.
• Ability to accept the philosophy and goals of the Agency.

Responsibilities

• Provides services according to the plan of care.
• Makes initial evaluations and regular reassessments of patient needs.
• Prepares clinical notes and coordinates services.
