TheraCare Home Health in Fort Worth, TX is seeking an RN Case Manager to oversee patient care supervision, discharge planning, and interdisciplinary coordination for home health patients. The role emphasizes leadership of LVN/HHA supervision and coordination with physicians and clinical teams.

The ideal candidate has RN licensure in Texas, 2 years of Home Health experience, strong communication and documentation skills, and reliable transportation to service patients in the Fort Worth area.

Qualifications

• RN licensed per state requirements.
• 2 years of experience in Home Health preferred.
• HomeCare HomeBase experience preferred.
• Good communication, assessment and documentation skills.
• Ability to read, write, and speak English.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Supervises and manages all aspects of patient care, including LVN and HHA supervision.
• Coordinates visits among SN, PT, OT, ST, MSW and HHA after Start of Care, Recertification or Resumption of Care.
• Initiates referrals to ancillary services like podiatry and community services.
• Facilitates interdisciplinary communication to optimize service delivery and patient progress.
• Leads weekly interdisciplinary case conferences for patient caseload.
• Develops care plans with realistic social and emotional goals for patients.
• Interprets and promotes patients’ rights and the Patients' Bill of Rights.
• Coordinates discharge planning with patients, clinical team and agency.
• Provides individual and group support for patients/families during adjustment or crisis.
• Completes required assessments, evaluations, visit notes and discharges per regulations.
• Functions as a team leader/role model and serves on committees as requested.
