Job Description

Are you an experienced RN, Home Health Case Manager in the Columbus, OH area? Then this position may be for you.

Shift: Mon-Fri 8am-5:00pm

Responsibilities

• Provide home care nursing to prevent hospitalizations and help clients remain independent at home.
• Demonstrate a high level of independence, clinical judgment, and strong clinical skills.
• Communicate effectively with a range of age groups.
• Care for clients such as pre-term infants with special needs, post-surgical patients needing follow-up care (e.g., antibiotics or wound care), individuals with chronic conditions (CHF, diabetes) requiring education, lab work or therapy, and those needing long-term medication management support.
• Educate, treat, and provide emotional support to help clients recover from injuries and illnesses.
• Maintain good communication, coordinate services, make referrals to other agencies/providers, and advocate for clients.
• Manage a caseload of 12-15 clients for a full-time RN.
• Travel within a 50-mile radius of Columbus with a reliable vehicle.

Territory

Far North of Delaware, far South of Lancaster, far West of London, far East to Newark.
