Transition Home Healthcare in Spring, TX is seeking a Clinical Branch Manager (RN) to lead our home health team and ensure delivery of high-quality interdisciplinary care. You will oversee planning, coordination, and delivery of services while maintaining compliance with federal and state standards.

The role requires supervising RNs, LVNs, HHAs and therapy staff, mentoring clinicians, reviewing documentation for accuracy and compliance, and driving continuous quality improvement across clinical

Qualifications

• Must have 2+ years experience as a Home Health RN Supervisor.
• RN in the State of Texas.
• Experience with HCHB EMR (preferred).

Responsibilities

• Supervises, monitors, and ensures competencies and performance of assigned home health care team(s).
• Oversees ongoing case management and ensures accurate evaluation and treatment plans.
• Reviews documentation for compliance with medical necessity and home health regulations.
• Drives quality improvement and outcome performance across the agency.
• Coordinates patient schedules and ensures efficient clinical operations.
• Mentors and educates assigned home health care team on documentation and standards.
