CommonSpirit Health at Home is seeking a skilled Registered Nurse with leadership capabilities to deliver high-quality home health care. You will develop individualized care plans, coordinate with a multidisciplinary team, and mentor LPNs, LVNs, and aides to ensure compassionate, patient-centered service.

You'll utilize advanced documentation tools to streamline care delivery, educate patients and families, and promote independence.

Qualifications

• Graduate of an accredited nursing program with an active RN license in the state served.
• Minimum of 1 year of nursing experience (Home Health experience is a significant plus!).
• Exceptional critical thinking, proven clinical expertise, strong case management abilities.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

• Empower and educate. Serve as a vital resource for patients and their families, providing crucial teaching and emotional support.
• Mentor and guide. Oversee your caseload, leading and nurturing LPNs, LVNs, and Home Health Aides to foster a strong, cohesive care team.
• Innovate with technology. Utilize leading-edge tools and best practices to optimize care delivery and maintain precise documentation.
