Job summary

Enhabit Home Health & Hospice is seeking a licensed professional to provide skilled home health care. You will educate patients and families, carry out physician orders, and implement the established plan of care in patients' homes.

Ideal candidates have at least one year of licensed experience, LIC in their state, CPR certification, and basic tech skills with mobile devices. A reliable vehicle and driving flexibility are required for in-home visits.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).

Responsibilities

Administer skilled care to patients requiring intermittent professional services. Teach the patient, family, and other members of the health care team. Perform services in accordance with the physician’s orders and the established plan of care (POC).

Education And Experience, Preferred

• Experience as a licensed professional may be deferred with a review and approval from the regional vice president or regional director.


• Medicare home health or hospice experience is preferred.

Requirements

• Must possess a valid state driver license


• Must maintain automobile liability insurance as required by law


• Must maintain dependable transportation in good working condition
  
• Must be able to safely drive an automobile in all types of weather conditions


• Must possess CPR certification for the healthcare provider
