Job summary

Enhabit Home Health & Hospice is seeking licensed clinicians to provide skilled home-based care across our service area. You will administer intermittent professional services, educate patients and families, and work within physician orders and care plans.

The role supports in-home hospice and health care delivery with a focus on patient comfort and quality of life. Candidates must be graduates of an approved school, hold state licensure, and have at least one year of licensed professional

Qualifications

• Graduate of an approved school of clinical education.
• Licensed in the state where they currently practice.
• At least one year of licensed professional experience.
• Basic technology skills, including operation of a mobile device.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with physician orders and the established plan of care (POC).

Responsibilities

Administer skilled care to patients requiring intermittent professional services. Teach the patient, family, and other members of the health care team. Perform services in accordance with the physician’s orders and the established plan of care (POC).

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education And Experience, Preferred

• Experience as a licensed professional may be deferred with a review and approval from the regional vice president or regional director.
• Medicare home health or hospice experience is preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification for the healthcare provider
