Job summary

Enhabit Home Health & Hospice is a leading home-based care provider delivering skilled nursing and supportive services to patients in community settings. The role emphasizes patient- and family-centered care, aligning with physician orders and care plans.

Qualified candidates must be licensed in the practice state, possess CPR, and have at least one year of professional experience, with Medicare home health or hospice exposure preferred for advancement.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.
• Experience as a licensed professional may be deferred with a review and approval from the regional vice president or regional director.
• Medicare home health or hospice experience is preferred.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).

• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
