Capital City Nurses, a Care Advantage Inc. brand, seeks an experienced Registered Nurse Home Care Supervisor to provide clinical leadership for Wilmington DE and Georgetown DE areas.

Lead, mentor, and advocate while ensuring high-quality care for patients at home and supporting caregivers. The role emphasizes supervisory visits, care planning, and collaboration with physicians and the interdisciplinary team.

Qualifications

• Active RN license in VA (Compact license accepted where applicable).
• Associate Degree in Nursing (ADN) or higher.
• Minimum of one year of nursing experience.
• Strong leadership, communication, and interpersonal skills.
• Ability to work independently and collaboratively.
• Reliable transportation and willingness to travel throughout the service territory.
• Proficiency with electronic documentation and technology.

Responsibilities

• Conduct supervisory visits to observe care delivery.
• Monitor patient progress and clinical outcomes.
• Provide clinical guidance and support to caregivers, aides, and nursing staff.
• Complete and maintain accurate patient records and nursing documentation.
• Ensure continuity and quality of care.
