Job summary

Corewood Care is seeking compassionate nursing professionals to join a vibrant, woman-run home care company serving the MD/DC Metro area. You will assess client needs, develop plans of care, and coordinate with care teams while traveling to client sites.

The role emphasizes wellness checks, client education, and maintaining accurate clinical records. Prior home care experience and DC RN licensure are preferred; a supportive team environment is promised.

Qualifications

• RN license with DC RN credentials.
• Current CPR/BLS certification.
• Vaccination requirements including TB, COVID boosters, Hep B vaccination.
• Accredited nursing program graduation.
• Clear criminal background and driving record.
• Two professional references in health care.

Responsibilities

• Meet clients and perform initial service needs assessments (virtual or in-person).
• Conduct wellness checks and educate clients through wellness sessions.
• Develop and update care plans for new clients.
• Communicate with care managers, caregivers, office staff, clients and families.
• Coordinate with Director of Nursing for supervision of services.
• Provide client training on care as requested.
• Maintain clinical records in compliance with state and federal laws.
• Perform client and caregiver evaluations every 30, 90, or 120 days as needed.
• Train new and existing staff.
• Work with a warm, supportive team.

Responsibilities:

• Wanting to join a vibrant and dynamic team and to work with a well-respected company in the MD/ DC Metro.
• Meets new and prospective clients (inside and outside the community) and performs an initial assessment of service needs - This could be done virtual and/or in-person, depending on client's request.
• Ability to perform Wellness checks.
• Host informational wellness sessions for CWC clients, caregivers and other interested members.
• Develops a Plan of Care for all new clients and updates as needed.
• Communicates with care managers, caregivers, office staff, clients, family members and other medical professionals about client's needs and concerns.
• Consults with the Director of Nursing regarding the supervision of services.
• Provides training to the clients regarding their care, when requested.
• Assures proper maintenance of clinical records and documentation in compliance with State and Federal laws.
• Performs client & caregiver evaluations every 30, 90 or 120 days as needed.
• Trains new staff and existing staff.
• Enjoys working with a warm and supportive team.

Qualifications:

• RN License –DC RN License
• Current and valid CPR / BLS Certification
• TB vaccination, Full COVID vaccination and MUST BE BOOSTED and proof of Hep B vaccination
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
• Clear criminal background and driving record
• Two professional references within the health care field
• Prior home care/ care management experience is strongly preferred
• Self-starter with the ability to work independently and manage time effectively
• Strong interpersonal skills, including a willingness to teach and mentor caregivers
• Comfortable with electronic records and MS Office Suite
