Job summary

Mercy Home Health is seeking a dedicated RN to provide skilled nursing in patients' homes across the St. Louis area. This per diem role offers day shifts and a flexible schedule, focusing on compassionate, patient-centered care and documentation in Epic.

Eligible candidates must have Missouri RN licensure, a valid driver's license, and BLS through the American Heart Association. Mileage reimbursement is available as part of the role.

Qualifications

• One year or more of experience in home health or related setting.
• RN license in Missouri; valid driver’s license and proof of auto insurance.
• Current BLS certification through the American Heart Association.

Responsibilities

• Provide day-to-day patient care in home settings, including multiple patients with varying acuity.
• Documentation and care planning using the Epic system.
• Manage admissions, recertifications, discharges, and patient teaching.
• Perform dressing changes, IV therapy administration, and site care as needed.

Provides day- to-day patient care in the home setting which includes, but is not limited to: case management and total care of multiple patients with different levels of acuity, admissions to home care services, post-hospitalization assessments, recertifications, discharges, patient teaching, dressing changes, IV therapy administration, access site care and general skilled nursing according to policy and procedure, assists with other needs that may be apparent in the home to assure safety and appropriate care i.e. lifeline, meals on wheels, and instruction on public programs availability. Provides appropriate and timely documentation by utilization of the Epic computer system.

Per Diem position provides visits on a as needed basis following all department policies. Performs duties and responsibilities in a manner consistent with our mission, values and Mercy Service Standards.

Qualifications:

• Experience: Prefer one year of experience in a related field (home health, acute care, etc.).

• Education: Graduation from an accredited nursing program required.

• Licensure: Current RN license in the state of Missouri required. Must also have a valid driver’s license and proof of auto insurance.

• Certifications: BLS Certification required through the American Heart Association.
