Conduct initial assessments; establish plan of care and provide prescribed treatments and demonstrated nursing abilities; coordinate the delivery of 24-hour care;Communication. Counsel and guide individuals and families towards self-help in recognition and solution of physical, emotional and environmental problemsEvaluation & Documentation. Conduct reassessment of each patient on a regular basis; maintain complete and accurate clinical documentation; supervise and serve as a resource to all home care staffREQUIRED QUALIFICATIONS:Nursing Diploma requiredLicense and current registration to practice as Registered Professional Nurse in New York StateCurrent NYS Driver’s licensePREFERRED QUALIFICATIONS:6 months- 1 year experience conducting UAS-NY Community Assessment preferred.Bachelors degree of Science in Nursing preferred.1 year of experience working with a frail or elderly population preferred.Availability of Motor VehicleEDUCATION:AS: Nursing (Required)LICENSES / CERTIFICATIONS:BLS - Basic Life Support - American Heart Association (AHA), RN - Registered Nurse - NYS Office of ProfessionsPHYSICAL REQUIREMENTS:Drive - Clinical - Roles in this category require frequent driving combined with mobile clinical care across settings such as emergency response, mobile clinics, and home health environments. Staff must repeatedly enter and exit vehicles, occasionally lift and carry equipment weighing up to 100 lbs. or more, and perform care within patients’ homes or other confined, unpredictable, or uncontrolled environments. These positions demand strong fine and gross motor control, firm grasping, and heightened visual and auditory awareness to ensure safe and effective care delivery. Emotional and cognitive demands are substantial due to rapid decision‑making, direct exposure to critical or sensitive situations, and independently managing patient care in the field or home setting. Environmental exposures may include traffic hazards, infectious agents, extreme temperatures, variable home conditions, and hazardous materials. It may be modified in the future and eligible for additional pay components. Individual annual salaries/hourly rates could vary based on position details and FTE.
