Maricopa County Department of Public Health is seeking an RN Case Manager to coordinate Early Intervention and Transitional Case Management for HIV-positive clients released from jail, including discharge planning and linkage to community-based HIV care. You will provide counseling, health literacy and care coordination to promote health equity within the jail and community.

Formación

• Two years of Registered Nurse experience.
• Valid unrestricted license as a Registered Nurse by the Arizona Board of Nursing (AZBN).
• Bachelor’s degree in nursing from an accredited institution.
• Equivalent combination of post-secondary education and/or nursing experience may substitute.

Responsabilidades

• Coordinate Early Intervention and Transitional Case Management services.
• Discharge planning and linkage to HIV care and community services.
• Provide counseling, health literacy and care coordination for HIV-positive clients.
• Maintain documentation in medical records and CAREWare; ensure HIPAA compliance.
• Collaborate with CHS staff, jail personnel and community providers.
• Respond to emergencies and support public health initiatives.
