Job summary

Legacy Community Health seeks a compassionate RN – High Risk OB Care Manager to join its maternal health team in Houston. This role focuses on high-risk perinatal care coordination, transitions-of-care, and triage for pregnant and postpartum individuals with complex medical, behavioral health, or social needs.

You will work with physicians, APs, Care Management, and Perinatal Navigators to navigate complex care pathways, aiming to reduce obstetric complications and improve postpartum follow-up

Qualifications

• Graduate of an accredited RN program.
• Current RN license in Texas and CPR/BLS certification.
• Minimum 3 years RN experience; OB inpatient experience preferred.

Responsibilities

• Provide RN-level triage for obstetric-related calls and messages.
• Manage a caseload of high-risk prenatal and postpartum patients.
• Coordinate transitions of care and follow-up care plans.
• Educate patients on warning signs, care plans, and self-management.
• Document assessments and communications in the EHR with accuracy.
