Job summary

Mosaic Management is seeking a Health Services Manager in Oregon to lead clinical operations across assisted living communities. The role requires an unencumbered RN license and at least 3 years of progressive clinical leadership in memory care, assisted living, or long-term care.

You will oversee resident assessments, care plans, medication management, staff coaching, and regulatory compliance, ensuring safe, high-quality care and coordinated communication among care teams.

Qualifications

• Unencumbered RN license in the state of employment.
• 3+ years of progressive clinical leadership experience in assisted living, memory care, or long-term care.
• Proficiency with EMRs, MARs, and Microsoft Office Suite, regulatory compliance, and medication management.
• Strong working knowledge of state licensing survey processes.
• Experience coaching, mentoring, and building high-performing clinical teams.

Responsibilities

• Provide leadership for all resident care services and clinical operations within the community.
• Participate in resident assessments, move-ins, and service plan reviews in compliance with Mosaic policy and state regulations.
• Review resident health records, monitor weights, and ensure timely response to changes of condition.
• Maintain updated internal service plans and ensure staff implement care appropriately.
• Document key resident updates daily in the community communication log to ensure seamless coordination between care teams.
• Oversee all medication delivery systems to ensure safety, accuracy, and regulatory compliance.
