Responsibilities

• Medication management: transcribe orders, coordinate with pharmacy, safely administer medications.
• Precise documentation: record daily patient care, observations and responses in the EHR per standards.
• Care planning: collaborate with the interdisciplinary team to implement individualized care plans.
• Communication & support: educate residents and families about care plans and safety policies.
