Job Description

Position Purpose: The primary purpose of the Registered Nurse is to provide direct nursing care to the residents and to monitor the day‑to‑day nursing activities performed by nursing assistants. The performance of these duties must be in accordance with the current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required and communicated by the Director of Nursing (DON) or Administrator to ensure that the highest degree of quality care is maintained at all times.

  • Adhere to St. Francis Health Services’ Mission and Core Values while performing company business.
  • Maintain confidentiality of company and resident related information at all times.
  • Serve on, participate in, and attend various committees of the facility as appointed by the Director of Nursing or Administrator.
  • Perform administrative requirements, such as completing necessary forms, reports, etc. and submitting such to the Director of Nursing or Administrator as required.
  • Participate in quality improvement activities as delegated.
  • Identify specific standards that contribute to rating.
  • Provide direct resident care and monitoring, including assessing physical and emotional condition and recognizing need for immediate attention.
  • Report all grievances and complaints made by the resident, family, or visitors to the appropriate supervisor following the facility’s established procedures.
  • Follow and recommend updates to the plan of care for residents, including immediate and long‑term goals.
  • Initiate plan of care at time of admission for each resident, review care plans and recommend revisions as needed.
  • Maintain knowledge of current nursing care plan for each resident and inform the nursing staff of objectives and/or goals, psycho/social needs, and physical needs.
  • Monitor nursing care to assure it is in compliance with the care plan.
  • Document changes in resident medical condition, incidents, responses to treatment and resident and/or family concerns.
  • Perform basic nursing procedures safely and efficiently and maintain technical skills, in accordance with facility policies and procedures.
  • Ensure medications are safely administered as ordered by the physician, knowing dosage, effects, and potential side effects.
  • Observe and document resident’s response to medication (especially PRN medications, new or adjusted doses).
  • Follow established procedures for drug control and comply with state and federal regulations regarding narcotics.
  • Maintain a complete clinical record of each resident’s stay and complete pertinent daily charting accurately.
  • Monitor resident’s needs and initiate appropriate intervention, notifying physicians, DON, Administrator and other departments as necessary.
  • Take direct appropriate action in emergency situations and coordinate care of any acutely ill residents.
  • Communicate clearly and appropriately, both verbally and in writing, with physicians, families, nursing assistants and other departments.
  • Teach the resident and/or family about the resident’s illness and educate staff on general knowledge of resident’s conditions and needs.
  • Demonstrate the knowledge and skill necessary to provide care, based on physical, psycho/social, educational, safety and related criteria for adult and geriatric residents.
Qualifications

Education and Experience: Must possess a current, unencumbered, active license in Minnesota as a Registered Nurse (RN). Maintain current certification in Cardiopulmonary Resuscitation (C PR).

Specific Requirements: Must be able to read, write, speak, and understand the English language while conducting company business, follow oral and written instructions, adhere to the Corporate Compliance Program Code of Conduct, company policies and procedures, and maintain a positive and friendly attitude. Must solve diverse procedural problems, communicate directives to others, coordinate with other departments, families and the general public, respect residents’ personal and property rights, and work harmoniously with other personnel and departments. Must be willing to work beyond normal working hours when necessary, and be willing to seek out new methods and principles and incorporate them into nursing practices. Must possess a willingness to request and/or accept direction/education as needed, and have a willingness to incorporate new methods.

Equipment and Procedure Proficiency: Glucose testing, hemoculture testing, dressing changes, nebulizer treatments, oxygen administration, medication administration, metered‑dose inhalers, tube feedings, care of ostomy and urinary catheters, ear lavage, suction machine, negative‑pressure vacuum systems, tracheotomy care, diabetic nail care, laboratory venipuncture and IV therapy, facility bathing tubs/showers, facility mechanical lifts, charting, transfer belts, specialized chairs, heel chairs and positioning equipment, dental care, hearing aids, alarms and other safety interventions/devices; light systems; walkie‑talkies and/or pagers (if provided by facility).
