Job summary

Living Branches in Pennsylvania seeks an Independent Living Nurse (RN) to coordinate nursing assessments, wellness services, and clinic operations for Residential Living residents.

You will obtain physician orders, manage care transitions, and document in PCC while collaborating with providers and the Director to expand wellness programs and services.

The role requires an active RN license and 2–4 years of nursing experience, with preference for geriatric care; HIPAA knowledge is essential.

Qualifications

• RN license (active).
• 2–4 years of nursing experience required.
• Experience with geriatric populations or dementia preferred.
• Knowledge of HIPAA and long-term care regulations.
• Ability to build trust and collaborate with team members.

Responsibilities

• Completes updated assessments for Residential Living residents annually or after health changes.
• Coordinates decision-making with residents and families regarding transitions in the continuing care community.
• Obtains physician orders for medications, labs, therapy, and other services as needed.
• Conducts weekly walk-in clinic in the Wellness Center, Mon–Fri.
• Maintains nursing records in PCC and documents services rendered.
• Coordinates in-home support services with internal and external providers.
• Supports wellness education programs and expands service options with the Director.
