Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) to join our home health visit team serving Medford and Somerville. The RN will provide skilled nursing and rehabilitative care, enabling clients to remain at home and collaborate with a multidisciplinary team.

The role emphasizes independent clinical decision-making, wound care, medication reconciliation, and flexible scheduling across the Medford/Somerville area.

Qualifications

• A current nursing license in good standing in the state of Massachusetts.
• Graduation from an accredited nursing program, confirmed by school transcript or diploma.
• At least one year of hands-on medical experience with adult/geriatric populations in acute care, emergency, home health, hospice, ICU, med surg, sub-acute or rehab within the past two years.

Responsibilities

• Triage and manage urgent clinical situations independently in the home setting.
• Perform head-to-toe assessments and monitor subtle changes in patient condition.
• Perform medication reconciliation, teaching, and safety monitoring.
• Conduct comprehensive wound assessment, care, and documentation.
