Qualifications

Responsibilities

• Deliver patient care incorporating Relationship Centered Care principles.
• Assess and evaluate patient care needs using critical thinking.
• Integrate assessment and intervention skills in nursing delivery.
• Collaborate with interdisciplinary team on care plans.
• Prioritize teaching and planning in patient care.
• Educate patients and caregivers about plans of care and health promotion.
• Communicate effectively with patients, families, and care team.
• Manage assignments within the Care Delivery Model of Modified Primary Nursing.
• Perform charge nurse duties as assigned.
• Document nursing practice to support quality and performance improvement.
