Qualifications

• RN license in Indiana is required
• CPR certification must be current or obtained within 90 days of hire
• License in good standing with no abuse/neglect findings

Responsibilities

• Assure that effective quality nursing care is delivered focusing on outcomes
• Identify needs and input for discharge planning and follow-up
• Collaborate with resident and care team to develop individualized plan of care
• Implement plan of care consistently and cost-effectively with resident-centered outcomes
• Collaborate with physicians and health care professionals by sharing information on changes in plan of care
• Deliver and maintain optimum resident care and comfort using current nursing practices
