Responsibilities

• Provide compassionate, high-quality geriatric care and build meaningful relationships with residents and staff.
• Assess and document changes in residents' conditions and respond to changes in health status.
• Collaborate with nursing and other facility teams to ensure coordinated and comprehensive resident care.
• Promote teamwork within the care team to meet residents' needs.
