Job summary

Preferred Home Health Care & Nursing Services seeks a Registered Nurse (RN) to assess, plan, implement, and evaluate patient care using evidence-based practices and multidisciplinary collaboration.

You will perform comprehensive assessments, develop individualized care plans with patient and family input, delegate care, and monitor outcomes while maintaining accurate documentation and regulatory compliance.

Qualifications

• Active RN license; compact license transfer within 60 days.
• BLS certification required.
• BSN preferred from an accredited program.
• Strong patient care, teamwork, documentation, and communication skills.

Responsibilities

• Coordinate and delegate care delivery with the care team.
