Job summary

Piedmont Henry Hospital is seeking a Diabetes Educator to deliver direct patient care in acute and ambulatory settings, based on medical staff requests. You will collaborate with the broader care team to prepare patients for discharge and provide homecare education to patients and families.

The role requires GA RN licensure or multi-state licensure, and Certification as a Diabetes Educator (CDE/CDCES) or Dietitian registration as applicable.

Qualifications

• Nursing licensure in GA (RN) or multi-state licensure.
• Certified Diabetes Educator (CDE) or Diabetes Care and Education Specialist (CDCES) required.
• Registered Dietitian (RD) registration required or implied by job postings.

Responsibilities

• Provide direct patient care in acute and ambulatory settings based on consults from medical staff.
• Collaborate with medical and nursing staff to prepare patients for discharge safely and efficiently.
• Provide homecare tools and skills to patients and families.
• Advise hospital staff on changes in diabetes management and educate staff and community.

Education

Graduate from a nursing program

Bachelor’s Degree in applicable field
