Job summary

Responsibilities

• Provide diabetes care and education within the Norton Healthcare System.
• Manage routine diabetes care and instruction on insulin and anti-hyperglycemic agents.
• Deliver comprehensive diabetes care education on nutrition, basic management, and lifestyle modification.
• Collaborate with multidisciplinary teams and external programs to provide wellness options; deliver on-site programs/classes via in-person or remote modalities.
