Masonicare at Mystic in Mystic, CT is seeking an RN Designee (ANC) for full-time day shifts. The role requires leadership in supervision of RCAs and LPNs, and conducting resident assessments to ensure quality care.

The ANC will coordinate with SALSA and other professionals, oversee nursing services 24/7, and support education for residents and families. CT RN license is required.

Qualifications

• RN degree or equivalent required.
• BSN preferred for higher role responsibilities.
• Two years of full-time clinical experience (or four years with diploma/associate).
• CT RN license in good standing required.

Responsabilités

• Performs initial and ongoing nursing assessments to establish resident care plans.
• Develops, implements and evaluates resident plans of services.
• Manages medication administration and interprets physician orders.
• Transcribes and follows physician orders in a timely manner.
• Education, supervision and counseling for residents, families on AL procedures.
• Coordinate plan of services and maintain continuity of care with other professionals.
• Document all information accurately and promptly.
• Supervise RCAs, LPNs, and other care staff as required.
• Organize educational programs, in-services and workshops to enhance skills.
• Assess client condition regularly and adjust service plans as needed (every 120 days or more).
• Refer to appropriate professionals when needed and coordinate home health services when appropriate.
• Plan for retirement of services when clients no longer require AL services.
• Ensure 24-hour nursing coverage and on-call support with SALSA teams.
• Provide nursing services including client teaching, wellness, health promotion, and disease prevention.
• Oversee delegation of self-administered medications as appropriate.
• On-call responsibilities shared with SALSA; flexible for all shifts.

The ALSA Nurse Coordinator (ANC) is responsible for providing assistance with leadership, management and assessment skills for the SALSA RN.

The ANC assists with managing the resident care services that are provided in the community. The management part of the ANC will include supervision of the RCAs and LPNs, resident assessments, and management, as defined by the SALSA.

Essential Duties and Responsibilities:

1. Performs initial and ongoing nursing assessments utilizing the nursing process to establish a comprehensive goal directed resident plan of services.

1. Develops, implements and evaluates resident plan of services.

1. Medication management and physician order follow through.

1. Transcribes, implements and evaluates physician orders in a timely manner.

1. Provides education, supervision and counseling to resident and family regarding AL procedures and other care needs.

1. Provides coordination of the plan of services and maintains continuity of care with other professionals.

1. Documents accurate, pertinent and comprehensive information in a timely and concise manner.

1. Conducts supervisory visits of resident care assistants, companions and homemakers as required.

1. Arranges and participates in educational programs, in-services, meetings and workshops and utilizes current literature to enhance professional skills and knowledge.

1. Assessments, completed as often as necessary based on client's condition but not less frequently than every one hundred and twenty (120) days, and prompt action when a change in the client's condition would require a change in the client's service program.

1. Referral to appropriate professionals or agencies, whenever the client's condition necessitates, including the provision of current clinical information ensuring that if the client's condition is no longer chronic and stable, services of a licensed home health agency are engaged or other appropriate arrangement are made.

1. Planning for clients who shall no longer receive or require the services of the assisted living services agency.

1. Implementation or delegation of responsibility for the availability of nursing services on a twenty-four (24) hour basis.

1. Nursing services, which shall include, but not necessarily be limited to:

a. client teaching

b. wellness teaching

c. health promotion

d. disease prevention.

1. Responsible for medication administration and delegation of supervision of self-administered medications as specified in subdivision (4) of the subsection.

1. On call responsibilities shared with SALSA.

1. Flexibility to work open nursing shifts when needed to include all 3 shifts.

1. Perform other duties as assigned.

Minimum Qualifications:

Education: RN Degree required

Experience: A Baccalaureate Degree in Nursing with two years full time clinical experience or a diploma or Associate's Degree in nursing with four years of full time clinical experience. Home Care, community health or Assisted Living experience a plus.

Certificates, Licenses, Registrations: RN License in good standing with the state of CT.
