ProMedica is seeking a Registered Nurse (RN) to join the Care Coordination team as the coordinator of a patient's plan of care. The RN will assess, diagnose, plan, implement, and evaluate care, working with patients, families, and the health care team to improve outcomes.

This per diem, remote position is limited to certain US states; Ohio license, CPR, and required certifications are needed. A Bachelor's in nursing is preferred, and benefits include 401k, EAP, and employee discounts.

Qualifications

• Current State license as a Registered Nurse.
• Current CPR training.
• Specialty certifications as required by individual nursing units.
• Ohio RN licensure requires twenty-four contact hours of continuing education during licensure period.
• At least 1 hour of contact hours must be related to Chapter 4723 of the Ohio nurse practice code and rules.

Location: Defiance Regional Hospital Department: Care Coordination Weekly Hours: 0 Status: Per Diem Shift: Days (United States of America) Pay Grade: $34.41 - $47.35 This is a remote position; however, employment is limited to residents of certain U.S. states due to legal, tax, and regulatory requirements. At this time, we cannot offer employment to individuals residing in California, Colorado, Hawaii, Massachusetts, New Jersey, New York, Oregon, Pennsylvania, Washington, or U.S. territories. Eligibility is based an applicant’s primary residence at time of hire.

Job Summary: The Registered Nurse (RN) is the core of ProMedica’s world-class patient care team. By providing patient-centered care through sensitivity and respect for the diversity of human experience, ProMedica nurses are dedicated to improving health outcomes for our patients and for the communities we serve. As the coordinator of a patient’s plan of care, you will interact with the patient, families and the health care team through open communication, mutual respect, and shared decision-making. • Ability to critique own daily practice for error and pursue corrective action.

Requirements

• Current State license as a Registered Nurse
• Current CPR training.
• Specialty certifications as required by individual nursing units.
• The RN in Ohio must complete twenty-four contact hours of continuing education during his/her licensure period.
• At least 1 hour of contact hours must be related to Chapter 4723 of the Ohio nurse practice code and rules.
• Maintains BCLS and other certifications required by the job, i.e., ACLS.
