Job summary

UnitedHealth Group is recruiting for an IPU Registered Nurse at Baptist Reynold's Hospice House, a partner in LHC Group. You will take full nursing responsibility for the Plan of Care for hospice patients and continuously evaluate performance to maintain high-quality care.

Primary duties include the initial nursing evaluation within 48 hours of referral, assessing physical and psychosocial needs, coordinating care with the IDG, and documenting patient progress daily.

Qualifications

  • Current unrestricted RN licensure in the state of practice.
  • Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
  • Current CPR certification or ability to complete within 90 days of hire.
  • 1+ years of clinical experience.

Responsibilities

  • Makes the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral.
  • Identifies the patient/family's physical, psychosocial, and environmental needs and re-assesses as needed, no less than every fifteen (15) days.
  • Initiates and coordinates the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition.
  • Documents problems, appropriate goals, interventions, and patient/family response to hospice care.
  • Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily.
  • Ability to function in any situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

As the IPU Registered Nurse, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Continuously evaluate personal and professional performance and makes necessary changes to increase productivity and quality of care delivered. Adheres to and supports the policies/procedures/goals/objectives of hospice to assure quality patient care.

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $37.00 to $49.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
