Job summary

Chapters Health System seeks a compassionate RN for our Hospice House in Florida. You will assess patient needs, coordinate the Plan of Care with the IDG, and provide palliative and supportive care to patients and families.

This role requires strong English communication and experience with EMR/EHR systems. Flexible scheduling and the ability to manage emotional stress are essential, along with lifting or assisting patients as needed.

Qualifications

• Current Florida license as RN or active multi-state/compact license.
• Minimum 1 year inpatient nursing or hospice experience preferred for external candidates; internal 6 months LPN with hospice/home health experience preferred.
• Ability to use EMR/EHR systems and communicate effectively in English.

Responsibilities

• Provide direct care to patients and families as part of the Interdisciplinary Group (IDG).
• Coordinate the Plan of Care reflecting evolving patient needs.
• Educate patients and families about disease, care, safety, and end-of-life processes.
• Report changes to IDG and participate in eligibility evaluations for hospice referrals.
• Attend deaths and prepare bodies for transport; assist with bereavement interventions.
