Qualifications

• 7 years of any combination of clinical, case management and/or disease management experience.
• Current State of PA RN licensure or multi-state licensure through the eNLC.
• Certification in Case Management preferred.

Responsibilities

• Maintain oversight over specified panel of members by performing ongoing assessments.
• Create care plans to address members’ identified needs and improve health outcomes.
• Ensure documentation compliance with regulatory requirements.
