Responsibilities

• Delivers care to complex patients through care management and care coordination.
• Conducts basic health assessments and provides direct and indirect nursing care.
• Escalates all pertinent clinical findings to the assigned APC within specified timeframes.
• Performs follow-up calls to ensure patient satisfaction.
• Assesses quality gap reports at each face-to-face visit.
• Ensures timely medical post-hospital discharge.
