Responsibilities

• Provide direct patient care to residents, including medication administration, wound care, and vital sign monitoring.
• Develop and implement individualized care plans in collaboration with our interdisciplinary team.
• Monitor and report changes in patient status to the nursing team and physicians.
• Coordinate with other departments, such as rehabilitation and dietary, to ensure comprehensive care.
• Maintain accurate and timely records and documentation.
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
