Qualifications

• Current, active, and unencumbered RN license in Vermont.

Responsibilities

• Perform nursing assessments and monitor residents' health status.
• Administer medications and treatments per physician orders.
• Develop, implement, and evaluate individualized care plans with the team.
• Monitor condition changes and communicate concerns promptly.
• Document assessments, medications, treatments, and responses accurately.
• Coordinate care with physicians, nursing staff, residents, families, and others.
• Support safe transitions of care and share resident information with the team.
• Use electronic health records to support documentation and care delivery.
• Follow infection prevention procedures and safety protocols.
• Promote dignity, privacy, independence and quality of life.
• Participate in quality improvement initiatives and outcomes.
